Primary Biliary Cholangitis (PBC)

Primary biliary cholangitis (PBC), formerly known as primary biliary cirrhosis, is a rare autoimmune liver disease that slowly damages the liver’s bile ducts.

In PBC, the immune system mistakenly attacks the small bile ducts inside the liver. This causes inflammation, scarring, and narrowing of the ducts, making it difficult for bile to flow properly. As bile builds up in the liver, it damages healthy liver tissue and can eventually lead to fibrosis (scarring), cirrhosis, and decreased liver function.

Understanding the Name

The name describes how the disease affects the liver:

  • Primary: Refers to a condition that begins on its own rather than being caused by another disease
  • Biliary: Relates to the bile ducts, which carry bile from the liver
  • Cholangitis: Means inflammation of the bile ducts

PBC vs Primary Sclerosing Cholangitis (PSC)

PBC and primary sclerosing cholangitis (PSC) are both diseases that affect the bile ducts, but they involve different areas of the biliary system.

  • PBC: Primarily affects the small bile ducts located inside the liver (intrahepatic bile ducts)
  • PSC: Can affect both the bile ducts inside and outside the liver (intrahepatic and extrahepatic bile ducts)

What Causes Primary Biliary Cholangitis?

The exact cause of PBC is unknown. Researchers believe it develops from a combination of:

  • An abnormal immune system response
  • Genetic factors
  • Environmental triggers

In people with PBC, the immune system mistakenly attacks the liver’s bile ducts, causing chronic inflammation and damage.

What Are the Symptoms of PBC?

Many people do not have symptoms in the early stages of PBC. The disease is often discovered during routine blood tests that show abnormal liver function. As PBC progresses, symptoms may develop.

Common Early Symptoms

  • Fatigue
  • Itchy skin (pruritus)

Later Symptoms

As liver damage progresses, symptoms may include:

  • Yellowing of the skin and eyes (jaundice)
  • Dry eyes and dry mouth
  • Pain in the upper right abdomen
  • Muscle, bone, or joint pain
  • Swelling of the feet and ankles
  • Enlarged spleen
  • Darkening of the skin
  • Weak or brittle bones (osteoporosis)
  • High cholesterol
  • Diarrhea, including greasy stools
  • Unintentional weight loss
  • Fluid buildup in the abdomen (ascites)
  • Fatty deposits under the skin called xanthomas*

*Xanthomas may appear around the eyes, eyelids, palms, soles, elbows, or knees.

How Is PBC Diagnosed?

PBC is often discovered during routine blood testing before symptoms appear.

  • Blood tests for specific PBC-related antibodies
  • Liver function tests
  • Imaging of the liver and bile ducts
  • Additional tests to evaluate the extent of liver damage

Early diagnosis allows treatment to begin before significant liver damage occurs.

How Does PBC Progress?

PBC typically progresses slowly over many years. The disease process follows this pattern:

Immune system attacks the bile ducts

Bile flow becomes restricted (cholestasis)

Bile buildup causes inflammation and liver injury

Ongoing damage causes fibrosis (scarring)

Advanced scarring may lead to cirrhosis and end-stage liver disease

Cirrhosis is permanent liver damage and may eventually require a liver transplant.

How Is Primary Biliary Cholangitis Treated?

There is currently no cure for PBC, but treatment can help slow disease progression, improve liver function, and manage symptoms. Treatment may include:

Medications to Slow Disease Progression

Certain medications help improve bile flow and reduce liver damage caused by bile buildup.

Symptom Management

Additional medications may be used to treat symptoms such as itching and improve quality of life.

Liver Transplant

If medications are no longer effective and liver function continues to decline, a liver transplant may be recommended. Liver transplantation can be an effective treatment option for advanced PBC, with many patients experiencing excellent outcomes. Regular monitoring, including imaging and laboratory testing, helps healthcare providers track disease progression and determine the best treatment approach.

What Is the Prognosis for PBC?

The outlook for people with PBC has improved significantly with earlier diagnosis and newer treatments. Many people with PBC are able to manage the disease for years with medications and regular medical care.

Fatigue, however, remains one of the most difficult symptoms to treat.

When PBC is diagnosed at a later stage, the disease may be more advanced and require more aggressive management. On average, progression to end-stage liver disease can take 15 to 20 years, although the timeline varies significantly from person to person.
