How to prepare for your appointment
If you have any questions, or to schedule an appointment, please call us at 214.947.4400 or 1.877.4A.LIVER. In order to provide excellent patient service please be prepared for your appointment.
- If you don’t have it already, download and print the New Patient Registration Packet.
- Please bring all completed paperwork with you to your appointment.
- Please arrive 30 minutes ahead of your scheduled appointment time.
- Please bring a current medication list of all medications you are taking.
- Please present your insurance card and your state identification card at each visit.
Special Note: Co-payments, deductibles, and coinsurance must be collected at the time services are rendered. We accept cash, personal checks, Visa, MasterCard, Discover, and American Express. *With the exception of certain satellite offices.
